• If additional testing is needed, it is $10, and only if warranted, such as a strep or urine test.

• Abscess with incision/drainage- $150
• PPD with reading- $75
• DMV handicap form- $115
• Tetanus shot- $115
• Personal office memo/letter- $70
• Surgical clearance with physical- $190

Injections prices: 

• B12, Ketorolac, Promethazine Injection- $25
• Dexamethasone injection- $30
• Ceftriaxone injection- $40
• Iron injection- $100

Semaglutide- Visit plus purchase of vial.  10mg/2mL vial is $300, no insurance needed.

• We accept Care Credit (adds 10% to visit) and all valid in or out of state insurances including all Medicaid.

Primary Care Services for everyone with or without insurance start at just $40 a visit!!

For those with insurance the visit is $60 for medication and lab orders, for those without insurance please see following:

Diabetes Treatment- $125 every 4 months

Thyroid Treatment- $110 every 4 months

Papsmear- $85 for insured and $200 for uninsured

Physicals for Adults- $90

Kids School Sports physical- $40

Kids School Sports EKG- $45

Florida Shots record- $50

Referrals to specialists- $100

Referrals for laboratories- $60

Referral for imaging- $55
